Patient Forms
Upon entering our office, a Team Member will welcome you as a member of our family. We will request that you complete our patient forms located in the New Patient Center. This paperwork provides us with your health history and information on your condition.

Next, you will have a consultation with Dr. Tagle to discuss your current health challenge (aka your areas of complaint). During this time, Dr. Tagle will determine whether or not chiropractic care can help you with your current health challenge.

After your consultation, Dr. Tagle will perform a focused oriented examination of your health challenge area/s, with standard orthopedic tests, muscle strength and flexibility testing. However, nothing will be done in our office without your consent.

Report of Findings
Based on the information collected and examination performed, Dr. Tagle will provide you with his opinion on what is possibly causing your health challenge and give you options on how we can get started.

Once you have made the decision to move towards feeling your best, your initial treatment will include a chiropractic adjustment. Depending on the severity of your health challenge, physical or massage therapy may be necessary in conjunction with your chiropractic adjustment. Should further adjustments be needed, Dr. Tagle will discuss what it’s going to take to getting you feeling your optimal best.

It will only take you 5 minutes of your time to get adjusted. Adjustments allow your body to function to its 100% capacity. We all have cell phones and computers that need periodic updates for them to run more efficiently and without those updates, it tends to run a little slow. Imagine your adjustments being the “updates” your body needs and without “adjusting” your spine, your body will eventually show signs or symptoms of something going wrong (ie. pain, numbness, tingling, weakness, etc).
